Why AI gets things wrong

Language models predict likely answers; they do not know when they are wrong. They can invent facts, misread an unusual document or apply a rule in the wrong situation. Giving the AI your own approved information to work from, and clear instructions on when to hand over to a person, reduces this a lot but never to zero.

Limit what the AI can do on its own: it can draft a refund email but not issue the refund, for example. Keep a log of every action so you can trace what happened. Sample and review output regularly, more often at the start. When something goes wrong, fix the cause, such as a missing instruction or outdated information, rather than just the single case.
